The employer sets an allowance.

For many planned episodes, the employer sets an allowance — typically between the 60th and 90th percentile of guaranteed prices in the local market. At the 70th percentile, 70% of local Care Groups cost less than the allowance.

Choose a Care Group at or below the allowance and out-of-pocket costs can be reduced to little or nothing. Choose above it; you may pay the difference — known before booking.
