Prior Authorizations for Medical & Pharmacy
Medical Prior Authorizations
For medical prior authorizations, please call (833) 246-5513 to confirm member eligibility and benefits before proceeding with prior authorization.
You must have the following information available:
- •Member name, DOB, and ID
- •Provider name and contact information (address and phone)
- •Diagnosis code
- •Procedure code
- •Dates of service
- •Type of Inpatient or Outpatient Service

Pharmacy Prior Authorizations
To submit a Prior Authorization request, you may call (877) 659-6101. Additionally, providers may submit a Prior Authorization request and medical records through the online service, CoverMyMeds.
